Lutheran Haven is looking for a compassionate and dependable Licensed Practical Nurse (LPN) to join our healthcare team on second shift. This role provides hands-on nursing care while helping create a safe, supportive, and positive environment for our residents. The LPN works under the direction of the Resident Care Coordinator and plays an important role in coordinating resident care, supporting staff, maintaining accurate records, and ensuring established safety and infection-control practices are followed.

What You'll Do

• Support Lutheran Haven’s Mission and Ministry through your daily work and interactions with residents and team members.
• Build positive working relationships with residents and coworkers while contributing as an effective member of the care team.
• Provide leadership, guidance, and encouragement to staff to promote high-quality resident care.
• Visit each individual resident three times per week, generally Tuesday, Wednesday, and Thursday, to identify needs, respond to concerns, and communicate requests to the Resident Care Coordinator.
• Oversee the day-to-day operations of the Assisted Living Facility, including employee supervision and resident care.
• Provide and receive shift reports at the beginning and end of each shift to ensure continuity of care.
• Complete initial rounds at the start of your shift and remain attentive to residents’ comfort, safety, and care needs.
• Promptly notify the supervisor or administrator on call of any situation requiring immediate attention.
• Provide personal hygiene, preventive, therapeutic, and rehabilitative services based on residents’ individual needs.
• Coordinate medical care with services provided by other healthcare disciplines.
• Complete admissions for new residents by assessing their needs and reviewing physician orders.
• Order medications, supplies, and equipment as prescribed and in accordance with established policies.
• Assist residents with self-administration of prescribed medications and perform approved nursing procedures and treatments within the scope of limited nursing services.
• Maintain the security of all medications, including narcotics, in accordance with facility policies and applicable regulations.
• Arrange and coordinate diagnostic and therapeutic services ordered by residents’ physicians.
• Maintain complete and accurate documentation regarding residents’ care, condition, and treatment in compliance with HIPAA.
• Accurately enter information from physician orders and medication records into the pharmacy Medication Observation Record when a resident is discharged or transferred.
• Perform other duties and responsibilities as assigned.

What You Bring

• Current Florida Licensed Practical Nurse (LPN) license.
• At least one year of verifiable, successful clinical experience.
• Experience working in a geriatric or rehabilitation setting is preferred.
• Current CPR certification.
• A team-oriented approach and a commitment to providing quality care to older adults.
